This is a 51-year-old patient who came to see me to discuss options to revise the results of breast augmentation that had been performed 18 years prior. The patient stated that she was initially very pleased with the results achieved during her primary breast augmentation, she noticed that her implants had started to become more laterally and inferiorly displaced and in particular, this patient missed the upper pole fullness that her breasts once had. She also had rippling which is common when saline implants are used on patient’s with minimal breast tissue.
We discussed capsulorrhaphy* with sutures versus using mesh with implant exchange. We also discussed using saline versus silicone implants. The patient liked to look and feel of the silicone implants and was comfortable with going this route. We did discuss the possibility of exchanging her implants for a larger size to give her the desired fullness and results that she wants without making her excessively large and using mesh to help support reinforce soft tissues to extend and enhance proper implant positioning.
During surgery, I removed her intact 350cc saline implants and used Galaflex mesh to support her new 415 cc Natrelle Inspira silicone gel implants. She is seen here before and 5 months after the procedure.
