This is a 44-year-old mother of two teenagers, standing 5'6" and weighing 128 lbs. She came in for a consultation to discuss the removal of her 13-year-old saline implants and the possibility of a breast lift (mastopexy). Her original surgery was performed by another surgeon with a transaxillary incision, meaning the implants were inserted via her armpit. Although she was not experiencing any direct issues with her breast implants, she felt ready to have them removed to avoid any future upkeep. She noted that she has had a persistent rash on her neck that has come and gone for the past 10 years or so and has gone to doctors/dermatologists and can get to bottom of it the etiology of the rash is unclear. She stated that while she does not think that the implants are the direct cause of the rash but she is interested in having the implants removed at this time. As a teacher, she desired to have her surgery of her summer break.
The patient was found to have intact smooth round moderate plus 300 cc implants. However, the right implant displaced 350 cc of saline, and the left implant displaced 400 cc. The implants were positioned subpectorally. The implant capsules were thin and free of abnormalities or periprosthetic fluid. It was also evident that the right breast had more natural soft tissue fullness compared to the left. To restore a natural, youthful appearance to her breasts after implant removal, I performed a superior pedicle mastopexy. Minimal breast tissue was excised, with the resected skin weighing 29 g on the right side and 27 g on the left. She is seen here six weeks post-surgery and is already thrilled with her results.
