This 25-year-old female patient, who lost 90 pounds through diet and exercise, presented with excess skin on her upper back and lower abdomen. Her highest weight was 275 pounds, and she currently weighs 188. Most of her weight loss occurred over three months in 2020, and since then, she has maintained her progress, fluctuating by only 10 pounds. She strength trains 3-5 times a week with heavy weights. As a preschool teacher traveling from Dallas, she scheduled her surgery during the summer break to allow for a smooth recovery.
Given her young age and the possibility of future pregnancies, I performed a tummy tuckwithout muscle plication, as well as excision of excess skin from the upper back. The resected tissue weighed 287 grams from the left upper back, 352 grams from the right upper back, and 929 grams from the abdomen. Exparel was used for 72-hour post-op pain management.
