This is a 33-year-old postpartum female presenting with glandular ptosis of the breasts. She breastfed her children for 8 months and 5 months, respectively, and was unhappy with the resulting skin laxity and ptosis. She liked the size of her breasts but not their position and did not want implants. As seen in her before photos, she had a slim 5’8”, 130 lb frame but presented with postpartum laxity of the abdominal wall, a large rectus diastasis in the central upper abdomen, and a very attenuated umbilicus.
She was a candidate for an auto-augmentation mastopexy with Galaflex mesh and tummy tuck with rectus diastasis repair. The auto-augmentation, or Ruth Graf flap, technique involves repositioning breast tissue to the upper pole of the breast to achieve a more youthful profile with better projection. She is seen here before and 3 months after surgery. The weight of the resected skin from the right breast was 21 grams, from the left breast was 15 grams, and the weight of the excised abdominal skin was 185 grams.
